Requirements to Become a Case Manager
To meet the State Division of Substance Abuse & Mental Health’s training standards for certification, all non-licensed individuals will be required to:
- Successfully pass a written examination which test basic knowledge, attitudes and case management skills
- Successfully complete a twenty (20) hour case management practicum over a two-week period
- All case managers must be familiar with Medicaid regulations pertaining to targeted case management
- Complete an application (either for Adult or Children’s case management) and either mail to Robert H. Snarr, Adult Program’s Manager, 120 N 200 W #209 Salt Lake City, UT 84103 or fax to 801-538-9892. (Please only send in application. CMHC keeps documentation of practicum and test results on file).
State Division of Substance Abuse and Mental Health Targeted Case Management Recertification Process
- Within three (3) years a minimum of (8) eight hours training must be completed specific to case management activities
- Complete recertification application (either for Adult or Children’s case management) and either mail to Robert H. Snarr, Adult Program’s Manager, 120 N 200 W #209 Salt Lake City, UT 84103 or fax to 801-538-9892.
**Changes to case management will be upcoming. However, until the CMHC’s are notified, the current process will be in effect.
